In March, 2025, TDLR emailed driving schools templates for new DE 964s and ADE 1317. Driving Schools were directed to begin transitioning to electronic forms rather than physical forms ordered from TDLR. Our video explains best practices in this transition. We also provide a zip file containing the templates, recommended file set up and an excel file for record keeping purposes.
